Quality Improvement and Patient Safety Leadership Program

The Quality Improvement and Patient Safety Leadership Program creates and supports health-care champions in the field of Quality Improvement and Patient Safety who can help to lead and facilitate change and improvements within their practice, their department, their hospitals, and across the entire health system. The program will be offered for the eigth year starting in October 2018.

Participants will be expected to undertake a project to improve quality or patient safety. To facilitate these projects, participants will be taught leadership and change management skills as well as learn about hospital and CLHIN resources that will aid them to complete the project by the end of the course in June. Considerable time will be devoted to the use of co-consulting groups so participants can learn through facilitated discussions with their peers.

$6,300 + HST


Beginning in October, the program covers 10 days of classroom-based activity, in 6 modules approximately 6 weeks apart. In conjunction with attending class days for the program, participants are required to complete a Quality Improvement project. The program concludes on the 10th day with a final presentation day.

How to Register

Who Should Attend

This program is designed for healthcare practitioners in Champlain LHIN healthcare organizations who have an interest in improving quality and patient safety. It is not necessary for participants to hold (or aspire to hold) formal leadership positions within the health system. 

To date, participants have included representatives of the Ottawa Hospital, CHEO, The Royal, the Champlain Regional Cancer Program, Hawkesbury & District General Hospital, Queensway Carleton Hospital, and the Peterborough Regional Health Centre, to name a few.

What You Will Learn

Through the process of developing and implementing a Patient Safety and Quality project, participants will also develop the following broad competencies and knowledge areas:

  • Healthcare: Understanding the importance of patient safety and quality projects to the Ontario health-care system
  • Leadership:  Understanding and working with different styles of leadership and using awareness of one's own style to exercise influence and decision-making
  • Project Management:  Understanding basic principles and elements of project planning, and execution in order to achieve meaningful results
  • Change: Appreciating the dynamics of change and learning how to communicate and lead change effectively, especially when working with other stakeholders in the health-care system
  • Performance Management: Learning essential tools for improving performance (business processes) and appropriate measures for results

Instructor Profile(s)

Lynn Davies, Academic Director

Lynn Davies is an organizational development consultant, facilitator, adult educator and coach. With 35 years experience as both a line manager and consultant to private and public sector organizations it has been her mission to help individuals, teams and organizations to learn, organize and manage for dignity, community and meaning.   She is a member of the faculty at the University of Ottawa’s Telfer School of Management Executive MBA with teaching specialties in Change Management and Team Development.

Her areas of expertise include: Change Management, Executive Coaching, Managerial/Leadership Development, and Organizational Development. A sampling of Lynn Davies' client list includes the Bank of Montreal, CSPS, DND, National Art Centre, Department of Justice, DNR, CSE, DINA, Health Canada, Mitel, Nortel, Cities of Ottawa, Kingston and Pembroke, the Ottawa Civic Hospital, the Ottawa and Carleton Boards of Education, and the Province of Ontario. Ms. Davies’ Masters Degree at Concordia University specialized in Human Systems Intervention. She has trained as a Gestalt Therapy Practitioner, a Certified Executive Coach and is a former board member of The Canadian Institute of Research and Education in Human Systems (CIREHS).

Samantha Hamilton, Academic Director: Quality

Samantha is a quality, safety and risk management professional with over 20 years of experience in a variety of health sectors. She has worked within the pharmaceutical, medical device, biologics, diagnostics, blood, long-term care and home care sectors and is currently working within the hospital sector. She holds an undergraduate degree in Biology (Microbiology specialization), a Masters in Health Administration and a Masters Certificate in Risk Management. She is holds a CPHQ designation and is trained as a lean six sigma black belt. She is experienced in quality management systems design, quality assurance processes, and a variety of quality improvement tools and techniques. She has presented at provincial and national conferences and has served on community health service boards and participated in provincial committees. In her current role she directs the Quality and Patient Safety team at The Ottawa Hospital.

Outside of work, Samantha is a mother of two small girls – Cate and Claire.  She and her husband Todd live in Stittsville and spend their time cycling as a family around their neighbourhood and maximizing family time wherever possible. 

Dr. Alan Forster, Medical Advisor

Dr. Alan J. Forster is a general internist and Co-Director of the Ottawa Hospital Center for Patient Safety. He is an Associate Professor of Medicine at the University of Ottawa and Scientist in the Clinical Epidemiology Program at the Ottawa Hospital Research Institute, where his research focuses on patient safety and quality improvement.

He has been the recipient of several prestigious awards recognizing his work within the field of health services research, including a Career Scientist Award with the Ontario Ministry of Health and Long Term Care and an Early Research Award from the Ontario Government's Ministry of Research and Innovation.

He has performed seminal work evaluating the incidence of adverse events following discharge from hospital. This work has led to the development of strategies to improve care during the transition home from hospital. He is also leading in the development of a hospital data warehouse, which will serve as a supporting infrastructure for a research program in patient safety and quality of care. He has over 35 publications in peer review journals. He has attracted close to $2 million dollars in extra-mural grant funding over the last five years to support his research program.

In addition to his significant contributions to a quality and safety program at the Ottawa Hospital, he provides ongoing scientific advice regarding quality of care to provincial and national organizations, such as the Ontario Health Quality Council and the Canadian Institute for Health Information.
